FW: Diabetes PATH Leader Trainings in N. Lower MI

 

 

All Area Agencies on Aging throughout Michigan are pleased to be included in the Michigan Health Endowment Funding www.healthendowmentfund.org This grant will allow the state of MI to expand evidence-based health and wellness programs, specifically Diabetes PATH (a disease specific focus of Stanford University’s Chronic Disease Self Management programs) and Matter of Balance.  

This will be a two-year process with many opportunities for organizations and individuals in our service and planning area to become certified Lay Leaders for Diabetes PATH, (Personal Action Toward Health in MI) and certified Coaches for Matter of Balance, a fall awareness & reduction program.  

In keeping with our mission, the targeted audiences for participants in both programs will be people 60 years of age and older.  

At this time, we are looking for interested individuals within Northern Lower MI counties who wish to become certified Lay Leaders/Coaches to teach the Diabetes PATH and/or Matter of Balance programs in their community.
